I just stumbled upon this article: "Mad Libs" Style Form Increases Conversion 25-40%

Quite amazing how a redesign can lead to a greater response. It makes sense though, a lot of forms on the web are boring. Making it a story or apply some creative styling makes it much less cumbersome to submit information.
It stresses once more you should design with the end-user in mind. What for you seems a one-minute form could be a real pain for the end user. Making it attractive, fun or offer something in return will increase returning visitors.
